At Apple A Day Convenient Care, we provide safe, affordable, and quality care to the various ages in the community of South Carolina. Our Board-Certified Family Nurse Practitioner provides individualized patient care across the board. We also offer Direct Primary Care for listed services.

Our focus is to keep our community healthy, which is why we’re committed to exceeding the expectations of our patients with innovative services. Our focus is to keep you well so you can continue to enjoy things you love. When you or your family member is sick, injured, or needs a physical, we strive for convenient access to care that is affordable.

We accept major insurance. We offer the most affordable self-pay health care in the area for people who are underinsured, uninsured or have high deductible plans.

Snehal Patel is a board-certified nurse practitioner with 13+ years of experience, committed to promoting community health and affordable healthcare access. Fluent in English, Gujarati, and Hindi, Snehal offers expertise in primary care, urgent care, and occupational medicine.
Snehal holds a Doctorate Degree in Nursing Practice from Chamberlain University, along with a Bachelor's and Master's degree in Nursing with a specialization in Family Practice, all obtained from Chamberlain University. Patel is a proud member of the American Association of Nurse Practitioners.

Yes, the information is completely secure. It is protected with a username and password / PIN number of your choice and through SSL encryption.
We share information with our patients that we feel can help them and keep them informed. Every practice is different, and we try to provide patients with key information regarding their health. For a complete list of features, ask our friendly staff.
Sign-up is simple and outlined in the steps above. You should receive a username and password from our practice via email that you can print out and customize after your initial login. Access can be made via any web browser, or through the smartphone app.
The app and our Portal allow you to reset your username and/or password. You can also call the front desk and we can reset the username and/or password for you.
User support is built into the web portal and the app, or you can ask a member of our staff.
DPC members pay a flat monthly fee in exchange for up to four visits, basic lab work, and personalized care. Because we cap the number of patients we serve, you receive longer, more meaningful appointments without the rush of traditional healthcare systems. Think of it as having the “all-access” pass to your doctor, not just an appointment when something goes wrong.

Yes, the provider will be available until 7 pm. We request that you only contact the provider for emergencies after hours. For non-emergency problems, please call our office on the next business day. For emergencies after 7 pm, please go to the nearest emergency department.
Yes, there is a one-time signing fee to join the membership. This fee will cover a comprehensive physical evaluation with basic lab work. This fee and the first month’s membership fee are due at that time.

The DPC model delivers comprehensive primary health care directly from your DPC provider. The patient pays for their care through a monthly fee similar to a Netflix Subscription.

Because DPC practices do not participate in any insurance or plans, the provider can avoid the ever-increasing and costly documentation demands and administrative intrusions that come with accepting insurance. Because DPC providers avoid these obligations to insurance companies, they are free to spend that time listening to and treating patients. With this, DPC providers eliminate the significant overhead costs of participating in insurance, and they can pass the savings on to you, the patient, and give compassionate, attentive, and timely care at an affordable cost.

If you have insurance through your job or by individual policy, you are often presented with the choices. For example, one option is lower or minimal, or zero dollar in cost for your monthly premiums, but you have to hit a high deductible before any coverage starts by your insurance.

With our DPC membership, utilize us for your basic health care needs. This saves you money every month, and your insurance becomes more like a catastrophic policy should a major issue arise.

You may cancel your membership at any time with 30 days written notice. Cancellation will take effect at the end of the current billing period. No refunds will be provided for payments already made.
We recommend that you have insurance to comply with Federal Law. Our medicine membership does not replace insurance, and ideally, we encourage our patients to carry a high deductible or major medical plan or a plan with a health savings account, thereby ensuring financial help should hospitalization or referral to a specialist be necessary. You should consult with your insurance provider to help you find a plan that works best with our medical model and that will meet your specific needs.

You CAN NOT use insurance for our services; however, if you would like, we can order labs, x-rays, and medications at the facilities preferred by your insurance. For example, if you would like us to order a blood test through your insurance, you will determine your copays and costs with the outside lab through your insurance and compare that with our in-house lab cost to pick a cheaper option.

Our providers will help coordinate care in the hospital, but patients will be cared for by a hospitalist – doctors who only see patients in the hospital. Our staff is open and available to consult with any hospitalist who seeks our assistance.

In the age of telemedicine, many conditions can be diagnosed and treated via a simple conversation by phone or video call. If appropriate, we will locate the nearest pharmacy and order the medication most suited for your circumstances. If you need to go to the hospital or seek other medical attention while away from home, we can advise you on that as well.
Although we accept FSA and HSA cards for payments, we encourage you to check with your plans and/or CPA if this is an eligible purchase.

Yes, however, your membership fees cannot be submitted to Medicare for reimbursement. You must have to sign a waiver every two years stating that you or your provider will NOT directly bill Medicare for your membership fees.

Medicare and supplemental insurance will still cover medical costs that you incur outside of Apple A Day Convenient Care. For example, prescriptions that you fill at the pharmacy, or labs run at an outside lab, or hospital charges, physical therapy, x-rays, and scans etc., are covered. You do not lose your Medicare coverage for other medical needs just because you are a patient of our medicine membership plan.

No. Patient privacy is a natural benefit of this medical model. We will never provide any third party with a copy of your records, unless you specifically ask us to do so, or if Ardane Medical is subject to a court subpoena or warrant.
Chronic pain, ADHD, and anxiety are serious medical problems that require specialized training to properly dose, monitor, and manage pain and/or anxiety treatment. Also, government agencies are requiring more regulations for providers who prescribe anxiety and/or pain medications, specifically any controlled substances. For these reasons, our provider at Apple A Day DOES NOT prescribe controlled substances or manage long-term pain or anxiety medications. There are specialists in our community who manage these serious conditions. We will help you to find the appropriate specialist to manage such conditions.
